The Sarnak Medallion Turn-In is a repeatable faction quest using medallions looted from sarnaks in Lake of Ill Omen. Hand them in to Iksar NPCs in Cabilis for shifts on Crusaders of Greenmist and Legion of Cabilis faction. This is the canonical Kunark-era access grind for any non-Iksar character who needs to walk through Cabilis without dying.

How to obtain the item

Sarnak medallions drop from sarnak warriors, scouts, and named sarnaks throughout Lake of Ill Omen. The fortress / tower area is the densest farming spot, with named sarnaks dropping rarer medallion variants that turn in for stronger faction. Drop rate on common medallions is moderate — bag a stack while leveling through the zone. Higher-tier sarnaks (the elite warriors and the tower-top mobs) drop the more valuable variants.

The zone is a credible 25-40 leveling spot, so most players accumulate medallions naturally during their Kunark leveling pass. Solo farming is possible from level 30+; small-group farming is faster and clears the fortress more efficiently.

Where to turn in

Hand in to Iksar NPCs in Cabilis. Different turn-in NPCs accept different medallion tiers — the basic medallions go to lower-rank Iksar guards, and the named-sarnak medallions go to higher-ranking Iksar faction-quest NPCs. Hail and confirm the dialog before each hand-in. The Last Camp's cabeast and cabwest zones both map to the same wiki page.

For non-Iksar characters whose Cabilis faction is too low to enter the city, run the first batch at the outer-Cabilis NPCs (the Iksar patrols outside the gates) until your faction is "apprehensive" or higher with the city guards. Once the inner-city guards stop attacking, you can reach the higher-rank quest-NPCs for better turn-ins.

Faction implications

Each turn-in raises Crusaders of Greenmist and Legion of Cabilis while lowering Sarnak Empire and Venril Sathir factions. This is the standard direction for any non-Iksar wanting to access Cabilis vendors and bank, or any character planning to run Iksar epics or Sebilis-era quest chains that touch Cabilis-aligned NPCs.

The reverse direction (Iksar character grinding Sarnak Empire faction) is rare but possible — kill enough Iksar guards or run anti-Cabilis quests instead. Most players want the +Cabilis direction.

The faction shift compounds quickly because medallions are a high-volume drop. Most players see "dubious" → "apprehensive" within one or two full stacks of common medallions, plus a handful of named-Sarnak medallions for the bigger jumps.

Variations and edge cases

A few notes:

  • Common vs named medallions. Different turn-in NPCs accept different tiers. The lower-rank Iksar guards take common medallions; the higher-rank quest NPCs take the named-Sarnak variants for stronger faction shifts.
  • Cabilis east vs west. Both halves of the city map to the same wiki page. Different turn-in NPCs live in each — verify location before traveling.
  • Drop rate variance. Common medallions drop at moderate rate; the named-Sarnak medallions are rarer and often contested at the fortress camps.
  • Faction window quirks. Crusaders of Greenmist and Legion of Cabilis may not appear in your faction window until your first turn-in surfaces them. Hand in one medallion to verify direction.
  • KOS Iksar guards block entry. If your starting faction with Cabilis is too low, the city guards will aggro before you can reach the turn-in NPC. Run medallions at the outer-Cabilis NPCs first to apprehensive before pushing inside.

The Last Camp any-race-class note

The Last Camp's any-race-any-class doctrine creates a real demand for this grind. A Human Shadow Knight, a Half-Elf Necromancer, or any other non-Iksar SK / Necro / Shaman who wants to use Cabilis as a base needs +Cabilis faction to walk through the city. The grind is mid-to-high-level (LoIO sarnaks can hit hard for an under-level character), so plan to run it after level 25. For Iksar characters playing canonically-evil-race classes like Cleric or Paladin, this same grind is what unlocks Cabilis vendor access for the unusual race-class combo.

Tips

  • Farm in the fortress courtyard. Highest density of sarnak warriors per pull.
  • Stockpile named medallions separately. They turn in for a much larger faction shift per item.
  • Watch for sarnak-zerker pulls. Multiple sarnaks aggro on adjacent kills — bring a puller or split spawns.
  • Combine with Iksar Crusader Bow Turn-In. Both feed the same faction.
  • Cabilis-east and Cabilis-west are one wiki page. Both halves accept turn-ins; check NPC location before traveling.
  • +Cabilis faction unlocks Sebilis-area NPC dialogs later in the Kunark progression.
  • Verify NPC dialog before each batch. NPC names occasionally drift across canonical patches — confirm the turn-in dialog references medallions explicitly.
  • Hand in one medallion first. Confirm direction and shift size before committing the full stack.
  • Bind near Cabilis. A bind point in Cabilis east or west cuts travel time on long farming sessions.
  • Watch for Sarnak captain spawns. They drop the named medallions and pop on regular timers — camp the fortress courtyard for both XP and named drops.
